To FAX or MAIL your order, please print out an exhibit booth reservation form or Program advertisement reservation form. To avoid duplication, please submit your information by one method only (if you use this online form, please do not also submit your information by mail, fax, or e-mail).

(1) EXHIBIT BOOTH RESERVATION

One booth with a full-page Program ad: $850 for the booth (Program ad is additional)
Additional Booths: $850 each.

Cancellation Policy: If we receive a written cancellation from you before December 5, 2008, we will refund the entire amount. From December 6 to January 2, 2009, we will retain a $200 service fee. Cancellations received after January 2, 2009 will not be refunded.

(2) PROGRAM ADVERTISEMENT RESERVATION

Advertisements should be sent by DECEMBER 5, 2008. PDF files attached to e-mail are preferred (sent to Karen Fricke, kfricke@aasianst.org).

Full-page ad (7.25" wide x 9.5" tall), $900.

Additional full pages, $850 each.

Half-page horizontal ad (7.25" wide x 4.5" tall), $575.

Second or Third Cover, $1,000.
